The Healthy Foundations Collective
FOR ADULTS WHO ARE DONE GOING IT ALONE
A structure that holds over time.
A community that keeps you accountable.
A light touch of support for the long haul of lifestyle change.
A year-round community health coaching program built around a quarterly rhythm, to keep you focused on the practices that matter over a timeline that fits real life, without demanding more than you can give. Available with or without a personalized clinical layer.
You want support that stays.
THIS IF FOR YOU IF…
Maybe you're new to this “healthy living stuff,” or just starting to explore it. You've absorbed enough to know your health needs real attention, but you haven't found a person or structure to support both the learning and the long, messy process of doing. A podcast is just information. A 12-week program for two thousand dollars is too much pressure.
Or maybe you've been at this a while. You've changed things, built momentum, felt the difference, and watched it quietly unravel when life took a turn. You know what to do. What you haven't had is a place to come back to when things slide.
Or perhaps you're already consistent with a lot, and you want practitioner support to dial in the details: making sure what you're doing addresses your vulnerabilities, your midlife changes, the parts you can't see on your own. That's what Collective + Care is for.
You might have worked on a few of the six pillars, but not all of them, and you can feel the gaps. In every case, what you need is the same: a structure that holds for years, not weeks, a community to witness you, and a container that doesn't dissolve.
The program ends.
The support goes with it.
WHY CHANGE DOESN’T HOLD
Most programs that don't stick aren't bad programs. The information is real, the plan is sound, and the results, while they last, are genuine. What isn't built to last is the container. When the program ends, the scaffolding that held the work in place goes with it, and it's easy to drift back to where you started. Not because you gave up, but because you're left with nothing to come back to.
But the container dissolving is only half of it. The other half is what you step back into: a culture that normalizes convenience over care, profits from your distraction, and constantly reinforces the defaults you're trying to change. You unravel because the support isn't there, culturally or structurally. And feeling isolated in your healthy choices is one of the biggest reasons they never become a lifestyle.
27-72%
Lack of social support shows up again and again in lifestyle medicine research as one of the primary barriers to sticking with healthy changes, cited by 27 to 72 percent of participants depending on the population and behavior. And yet most programs treat community as an optional bonus.
The details of a healthy lifestyle are deeply personal. No two people's ideal recipe or process will look the same. So building a lasting practice is ultimately yours to do, but the forces you're pushing against are shared. When we connect across this struggle, we develop both our personal and collective power.
In the Collective, community isn't the feel-good part or a free bonus. It's structural. It's the Social Connection pillar made real.
A model that's built to stay with you. For as long as it takes.
The Healthy Foundations Collective is not a 12-week program. It doesn't have an ending. This is the place you keep coming back to, month after month and year after year, as your understanding deepens, your consistency builds, and positive changes in your health compound.
A DIFFERENT KIND OF STRUCTURE

HOW IT WORKS
The Collective structure.
The Quarterly Retreat
Four times a year, we gather for a four-call coaching retreat over the course of a week: Reflections & Celebrations → Education → Planning → Commitments. This is the heartbeat of the Collective.
Between-Retreat Support Calls
Two community coaching calls per month keep the work moving and the connection alive between retreats. You're never more than two weeks from live support.
Community Thread
A private, HIPAA-compliant community space for ongoing connection. Drop in anytime to share struggles, questions, wins, and engage the kind of peer accountability that makes this journey sustainable.
Getting Started: The Healthy Deviant
New members receive a copy of The Healthy Deviant and complete a self-paced onboarding module before their first retreat. You arrive already in the work.
Private à la carte coaching sessions are available for purchase anytime.

Frequently asked questions
How is the Collective different from a regular coaching program?
Most coaching programs have an end date. The Collective doesn't. It's a membership that stays with you through the quarters when everything goes right and the ones when it doesn't. The quarterly retreat gives you a rhythm of coming back. Hit every goal or none of them, and you're still welcome at the table. Success here means coming back, taking stock, and adjusting.
What's the time commitment?
The quarterly retreat is four calls on Zoom over the course of a week, typically 60 to 90 minutes each. Community support calls run about 60 minutes, twice a month. The thread is there whenever you want it. You don't need to show up for everything. The bare minimum: post your quarterly commitments and a reflection on your progress each quarter, so the community can recognize your efforts and feel you working alongside us. Even if you miss an entire retreat, those posts help us support you and hold you accountable.
How do I know if I'd be better in the Collective or Collective + Care?
The Collective is a good fit if you're generally healthy and want support building your foundational health, whether you're following your own wisdom or a plan from another practitioner.
Collective + Care is for you if you don't have a progressively minded practitioner to work with, or if your medical provider wants you to reach a specific goal you haven't been able to reach on your own: a lower HbA1c or blood glucose, a change in your cholesterol or other lipids, or another marker you're measuring and want to tailor your lifestyle toward. Your strategy session, follow-ups, and bloodwork will encompass that as a primary objective, along with your more personal objectives.
Do I have to complete the Foundational Health Assessment first?
Yes. Every One Body One World program starts with the Assessment. It's a $50 intake and 40-minute consult to make sure the Collective is the right fit and to show you where to begin. If you enroll within 30 days of completing it, the $50 is credited toward your membership.
How do I join the Healthy Foundations Collective?
Once you've completed your Foundational Health Assessment, if the Collective is the right fit, you'll get an invitation to your client portal on Practice Better, the platform the Collective runs on. Accept the invitation, verify or add your information, and ideally download the app. Inside your portal, you'll pay for your membership or start the payment plan, and sign your waivers and agreements. Then you're in, and new members can start onboarding right away.
From your portal (or the app), you can join community calls, post to the community thread, purchase private sessions, and message us anytime. Collective + Care members also schedule their strategy session and follow-ups there, and can upload past bloodwork or test results. The platform, our messages, and our calls are all encrypted and HIPAA compliant. To protect everyone's privacy, all communication happens through Practice Better.
What is the bloodwork in Collective + Care, specifically?
It's a comprehensive suite of foundational lab markers compiled with a functional sensibility. There's no esoteric functional testing. It's a broad-based suite of markers, designed to include a level of detail we often have trouble getting through standard medical practitioners. It's designed to inform your wellness priorities and identify your most important markers to track over time. These panels will be relevant and shareable with any other type of practitioner on your medical care team. If you live in New York or New Jersey, there's a $300 annual surcharge. Regulations enforced by these states' policies and Departments of Health demand additional fees on all direct-to-consumer lab services.
I'm on gender-affirming hormone therapy. Is prescriber support available to me?
You're welcome here, and I want you to have the full picture up front. I'm not licensed to prescribe, so all prescriber support comes through our partnership with licensed prescribers. Their services don't currently include midlife hormonal care for people on gender-affirming hormone therapy, and until they expand, we can't offer it. We're advocating for that expansion to include you. Everything else in Collective + Care is fully yours: the coaching, your strategy and follow-up sessions, the bloodwork, and prescriber support for other wellness needs.
Can I switch tiers?
Yes. If you're in the Collective and want the clinical layer, you can upgrade to Collective + Care, and what you've already paid is applied toward the cost. We don't offer mid-year downgrades. Much of Collective + Care's investment is front-loaded, with a longer strategy session and bloodwork early in your year, so it's best to choose a tier you can stick with for a full year. Switching tiers at renewal, in either direction, is easy.
What does renewal look like?
Memberships renew each year at a continuing-member rate: $400/yr for the Collective and $1,400/yr for Collective + Care. Renewal is automatic, and you can turn it on or off anytime in your client portal. That's also where you choose your tier for the coming year. Payment plans are available for renewals too.
Can I just get private coaching without enrolling in a program?
Not on its own. Vanden only offers coaching and all related services to people in the One Body One World community. If you're looking for private 1:1 coaching, that's available through the Lane Change Program.
It starts with one conversation.
